Cost:
$250 per month 36 week to 4 months postpartum
$200 per month 5 months to 1 year postpartum
Client will be sent recurrent billing
Client responsible for scheduling monthly visit
HSA/ FSA eligible
Superbill will be provided for eligible services
unused services will not be refunded
**Consultations outside of time allowed will be billed at normal rates in 15 minute increments. Mileage limits for in home consultations are within a 35 mile radius of 75701, services outside the radius will incur an excess mileage fee of $50
